CSC’s Healthcare Transformation Initiative builds infrastructure, creates awareness, and sponsors professional and community health education events. Across five distinct initiatives, we focus on educating communities, non-profits, payers, hospital systems, practices, and individual providers, allowing for the entire medical neighborhood to move in tandem towards the pursuit of a higher level of health care delivery and overall population health improvement. Recognizing the barriers which exist between individuals, their communities, and health care, we focus on educating all members of the medical neighborhood.
Leading the way for wide-scale change in healthcare transformation, CSC continues to provide the best possible healthcare transformation education and support within the primary care setting to optimize the healthcare experience for providers, practices and community members. To do this effectively, we focus on three primary functions:
Horizons on Healthcare Newsletter: This e-newsletter is focused on creating a new landscape for community and clinic health. Horizons is CSC’s first newsletter dedicated to collective impact in healthcare. The monthly e-newsletter features a calendar of upcoming health-related events, noteworthy publications, health literacy resources for community and clinic providers, along with healthy highlights from across Oklahoma.
Family Health Coalition Newsletter: This e-newsletter focuses its efforts on four areas: prevention, health and wellness, early development, and medical neighborhood development. Monthly contents include a robust calendar of health-related events and multiple continuing education opportunities.
Family Health Coalition: 35-40 trainings per year for the direct service community
Claim Your Coverage: 3-5 events per year to help community members enroll for healthcare coverage on the federal marketplace
County Health Improvement Organizations (CHIO): Ongoing Continuing Education for Oklahoma physicians
County Health Improvement Organizations: We support 23 organizations serving 26 counties in Oklahoma through community organizing, development and health improvement activities. This is done in collaboration with the Public Health Institute of Oklahoma.
Family Health Coalition: We support the family health community through capacity building and by serving as a network between maternal/child health providers.
Tulsa Healthy Start: We support and promote the Maternity Medical Home concept in public and private OBGYN offices across Tulsa County.
